Mobile Integrated Health - Bridging the Gap to Recovery
Mobility limitations. Medications. Scheduling follow-up care. There are a lot of details to remember when you leave the hospital. But all most patients want to do is relax and recover in the comfort of their own homes.
That’s why Reedsburg Area Medical Center (RAMC) and the Reedsburg Area Ambulance Service are pleased to offer a Mobile Integrated Health program to help bridge the gap between hospital discharge and recovering at home, allowing patients to focus on healing.
The Mobile Integrated Health program provides recent RAMC patients with an in-home visit from a paramedic within 48 hours of hospital discharge as well as four follow-up phone calls to provide further support for patients who need it.
Through the Mobile Integrated Health program, patients and their families receive the following support from paramedics:
- Medication review
- Assistance with making follow-up appointments with providers
- Health education on symptom awareness and how to best manage illnesses
- Home safety check
At this time, the Mobile Integrated Health program is available to patients within the Reedsburg city limits who have been diagnosed with congestive heart failure, chronic lung disease, pneumonia, stroke, diabetes, and heart attack.
It is our intention to continue growing this program so that we can serve patients with other diagnoses, inside and outside of the city of Reedsburg. This has been a great collaboration between Reedsburg Area Ambulance and RAMC, and together we look forward to helping our community live the most vibrant lives possible.
