Chronic Care Coordination Program
Nearly 3 in 4 people 65 years of age or older have multiple chronic conditions. These patients fill 93% of prescribed prescriptions and comprise nearly 80% of physician, ER visits and hospital stays.
We know that some patients require care management beyond the walls of the hospital or their personal doctor’s offices. That’s where Reedsburg Area Medical Center has stepped in to help!
RAMC now offers our Chronic Care Coordination program to assist Medicare patients ages 65 and older with two or more chronic health conditions. RAMC’s Chronic Care Coordination staff includes registered nurse Katie Kopp, seeing patients at the Physicians Group located on the hospital campus.
Katie offers these patients:
- a complete medication review
- a review of preventive health services that may be due
- provide education regarding the patient’s chronic illnesses
- Advance Care Planning
- a partnership with each individual patient to help them reach their best possible health
- and an annual Medicare wellness exam.
Through this program, Katie spends the time needed with patients to develop relationships, assess needs, problem solve, monitor progress and assist with connecting patients to local resources. In the end it should save patients time, money and hospitalizations.
This program requires a referral from a patient’s personal provider.
